New facility to integrate rehabilitation and acute care under one roof
Come December this year, Changi General Hospital and Saint Andrew's Community Hospital will start operating their first wards that integrate rehabilitation with the treatment of acute illnesses.
These wards will be housed in their new Integrated Building, which celebrated its topping-up ceremony today.
The facility is the first of its kind in Singapore, offering a new model of healthcare, especially for elderly patients.
Dr Lee Chien Earn, CEO of Changi General Hospital, says the traditional approach is for a patient's acute illnesses to be treated before rehabilitation starts.
"Now, we are trying to do rehabilitation and acute care, at the same time. We felt that there was a need to institute rehabilitation as well as to expedite the process, even while the acute illness is going on."
With the Integrated Building, CGH and Saint Andrew's Community Hospital, or SACH, will collaborate closely to roll out joint initiatives in clinical care.
For example, SACH's Home Care Services and the CGH Transitional Care team will be housed next to each other.
This will ensure more effective case management and coordinated healthcare for patients.
Dr Lee says the hospitals will also integrate the patient's caregivers into the team.
This will help them better understand how to care for their loved ones when they return home.
"When patients enter hospital, sometimes they virtually enter what I would call a Twilight Zone. When you enter hospital, you do a lot of things they don't do at home. So, it is not surprising why patients would find difficulty when they want to transit back to their own homes, because what they were used to in hospital is not available at home."
